Abstract
Introduction: There is as yet no consensus as to the effect of Ramadan fasting on fasting blood glucose. We carried out a study to help clarify the situation.
Methods: This was a semi-experimental (pre- and post-) study of 115 healthy volunteers (67 men and 48 women), who fasted for at least 25 days during Ramadan. Blood samples were taken 7 days before Ramadan (at 7am after a 8-hour overnight fast), and on the 14th and 28th days of Ramadan (1 hour before sundown). The mean duration of daily fasting was 11.5±0.5 hours. Plasma glucose was measured by an enzymatic assay. Statistical analysis was by the paired-t and ANOVA functions on SPSS10.0 software.
Results: Fasting plasma glucose in the group as a whole decreased from 88.4±9.0mg/dl pre-Ramadan to 75.4±15.3mg/dl on day 14 and 62.9±7.7mg/dl on day 28 (p<0.001). Both men [87.5±8.8mgdl (pre-Ramadan) 60.8±6.4mg/dl (day 28)] and women [89.7±9.3mgdl (pre-Ramadan) 65.7±8.4mg/dl (day 28)] experienced a significant decrease in fasting plasma glucose levels (p<0.001 in both). There was a reduction in calorie intake during Ramadan in every volunteer (p<0.001), and there was a direct correlation between reduction in calorie intake and fasting plasma glucose (p<0.01).
Conclusion: Fasting plasma glucose decreases with Ramadan fasting and is associated with a reduction in calorie intake. The decrease in plasma glucose does not seem to be accompanied by any serious adverse effects in healthy volunteers, however.

Abstract
Introduction: Urbanization, establishment of sedentary life style and unhealthy diet in association with environmental stress has led the cardiovascular risk factors to prevail in the metropolitan city of Tehran. Survey of lifestyle related cardiovascular risk factors is one of the priorities of Tehran university population laboratory in the 17th zone of Tehran.
Methods:1573 inhabitants of 17th zone of Tehran were recruited by one stage cluster random sampling according to the model of WHO MONICA project.
Results: 58.6% of men and 64% of women had BMI ≥ 25 kg/m2. 41.7% of men and 37.6% of women had hypertension. 8.9% of men and 12.2% of women had diabetes. 34.4% of men and 44.6% of women had total cholesterol ≥ 200 mg/dl. 34.1% of men and 32.6% of women had triglyceride ≥ 200 mg/dl. 34.7% of men and 4.2% of women were regular smokers. 21% of participants had positive family history of cardiovascular disorders.
Conclusion: The prevalence of hypertension in this region is high, that of dyslipidemia relatively lower than the other studies performed in Iran. The prevalence of Diabetes and overweight /obesity is high and comparable to other Iranian studies. It seems that the population of this zone is at high risk for stroke and then coronary heart disease. This fact must be considered in the future interventional programs to control cardiovascular risk factors in the region.

Abstract
Background: Cardiovascular disease is one of the major leading cause of death in Iran. There is a strong association between parental history of cardiovascular disease clustering of risk factors in the offspring. Detection and treatment of cardiovascular risk factors since childhood is essential to reduce the incidence of disease in adulthood. This study was performed to identify major cardiovascular risk factors in middle school-aged children and their parents in high risk compared to control families.
Methods: The middle schools of the 6th of Tehran were divided randomly into two groups. A total of 169 high-risk children with their families were recruited from the first group while 105 control children with their families were recruited from the second group of schools. Coronary risk factor survey was performed in the participants.
Results: Prevalence of increased total- and LDL-cholesterol and high FBS were higher in high-risk parents and children. Prevalence of increased BMI was higher in fathers and children of high-risk families. More fathers in high-risk families were smoker. The means of waist circumference and WHR were significantly higher in high-risk fathers. The means of total- and LDL- cholesterol were significantly higher both in parents and children of high-risk group. The means of FBS were significantly higher in fathers and offspring of high-risk families.
Conclusion: Cardiovascular risk factors are more prevalent and clustered in high-risk families. Screening of these families is essential to prevent progression of cardiovascular disease since childhood and reduce its burden in adulthood.

Abstract
Background: Plasma concentration of adiponectin was found to play an important role in regulation of glucose metabolism, insulin resistance and conditions commonly associated with obesity. The aim of this study was to assess adiponectin level and its relation with percent of body fat mass and insulin sensitivity in over-weight men.
Method: This study was conducted on two groups of type 2 diabetic (n=20) and non-diabetic (n=20) overweight adult men. Serum level of adiponectin (ELIZA), FBS (GOD-PAP), fasting insulin (immunoreactive assay) and body fat mass (Bio-electric impedance) were measured. Insulin sensitivity was calculated using QUICKI.
Results: Diabetics had significantly higher mean of fat mass% than non-diabetics (26.55± 2.87% vs. 22.93±2.64 p< 0.05), despite fairly similar BMIs (27.7 kg/m2). Adiponectin concentration was lower in diabetics (7.77 ±3.53µg/ml) than non-diabetics (8.13 ± 0.03µg/ml) however, this difference was not statistically significant. The negative correlation between adiponcetin and fat mass didn't reach statistical significance. Adiponectin positively related with insulin sensitivity in non-diabetics (r= +0.5 p= 0.04).
Conclusion: Lower level of adiponcetin in diabetics than their non-diabetic counterparts. As well as the positive association of adiponectin with insulin sensitivity confirm the suggested role of decreased level of adiponectin in conditions such as insulin resistance and diabetes.

Abstract
Background: One of the most common complications of Diabetic Polyneuropathy (DPN) is decreased balance and Postural Control disorder. Balance has an important role in static and dynamic activities as a base of activities of daily living. The objective of this study was the evaluation and comparison of dynamic and functional balance in patients with DPN and healthy subjects.
Methods: In this study, dynamic and functional balance of 11diabetic patients with DPN (detected by Michigan questionnaire) and 11 healthy subjects were evaluated by Biodex Balance System (BBS) and Tandem Stance (TS) test in open and closed eyes conditions. The participants were matched by age and BMI. An independent t-test was used to compare balance parameters between patient and normal group. Furthermore, the correlation between dynamic balance parameters of BBS and TS tests were calculated using the Pearson’s Correlation Coefficient.
Results: The results of BBS in patients with diabetic peripheral neuropathy were significantly higher than normal subjects (P<0.05). The mean of time of TS test in diabetic group was significantly lower than healthy subjects (P<0.05). Also there was a significant (P=0.004) negative (r = 0.794) correlation between Medio-Lateral stability index (recorded from BBS) and TS with open eye, in diabetes group.
Conclusion: Older adults with DPN have an impaired functional balance in comparison with matched control subjects and this may expose the patients to danger of falling during daily activities.

Background: Diabetes disorders can lead to muscular burnout. The aim of this study was to determine the simultaneous effect of resistance training and endothelial stem cell injection on the factor of degeneration in the muscles of diabetic rats.
Methods: 36 male wistar rats (age 6 weeks with weight of 20±200g) were randomly divided into five groups of basic healthy, diabetic control (D), diabetic and resistance training (DR), untrained diabetic by injection of endothelial stem cells (DI), diabetic training by simultaneous injection of endothelial stem questions (DRI). Western blotting and insulin resistance index were measured by ELISA method to evaluate the changes in MURF1 expression. The data were analyzed by two-factor analysis of variance by SPSS software version 19 at a significant level of α≥%5.
Results: In this study, 6 weeks of resistance training significantly reduced MURF1 and insulin resistance index. The interactive effect of resistance training and stem cell injection also resulted in a significant decrease in murf1 levels, but insulin resistance index was not significantly superior to only training or injection.
Conclusion: Based on the findings of this study, it can be stated that resistance training or injection of endothelial ancestral cells can improve muscular degeneration, but simultaneous use of these two strategies was not superior in reducing atrophy complications in diabetic rats.

Abstract
Background: Because insulin therapy cannot properly control the progression of diabetes and its complications, other alternative therapies may be desirable. The aim of this study was to investigate the effect of resistance training and endothelial stem cell injection on skeletal muscle oxidant and antioxidant status in STZ-induced diabetic male rats.
Method: In this experimental study, 36 male Wistar rats (age 6 weeks) were divided into six groups of control (healthy), basal diabetic control, diabetic control, diabetes + stem cell injection, diabetes + resistance training and diabetes + stem cell injection + resistance training. In this study, rats became diabetic intraperitoneally using streptozotocin as a single dose of 40 mg/kg. Resistance exercises including climbing a one-meter ladder with weights hanging from the tail were performed for 17 sessions. 500,000 bone-derived stem cells were injected by a cell counter. The levels of superoxide dismutase (SOD) and malondialdehyde (MDA) in skeletal muscle tissue of rat were measured by using the kit and ELISA method.
Results: The results showed that the SOD level of rats in the resistance training and endothelial stem cell injection group was significantly higher than the diabetic control group (P<0.001). Also, the level of MDA rats in the resistance training and endothelial stem cell injection group was significantly lower than the control group (P<0.001).
Conclusion: Resistance training and endothelial stem cell injections can be considered as a non-pharmacological treatment to reduce skeletal muscle complications in type 1 diabetes.

Abstract
Background: Cardiovascular disease is a major cause of death in patients with type 1 diabetes, and cardiovascular risk remains high even in patients with type 1 diabetes with good metabolic control. The aim of this study was to evaluate the effect of six weeks of aerobic exercise on inflammation and damage indicators of heart tissue in type 1 diabetic male rats.
Methods: In this experimental study, 19 male Wistar rats (mean weight 200-250 g) were randomly divided into four groups: aerobic training, sham, control and healthy. In this study, induction of type 1 diabetes was performed by injecting a single dose of streptozotocin dissolved in sodium citrate buffer intraperitoneally. Aerobic exercise program was performed with intensity of 50-60% VO2max, 5 days a week for 6 weeks. After anesthesia, an autopsy was performed and left ventricle of the heart was removed. Levels of Tumour necrosis factor α (TNF-α), Peroxisome proliferator-activated receptor-γ coactivator (PGC-1α) and Creatine kinase (CK) in rat heart tissue were measured by Western blotting. Data were analyzed by One-way ANOVA and Tukey post hoc test at the P<0.05.
Results: The results showed that six weeks of aerobic training led to significant decrease in TNF-α and CK and significant increase in PGC-1α of the heart tissue in type 1 diabetic rats (P<0.001).
Conclusion: According to the results, it seems that aerobic training can help improve the inflammation and damage indicators of heart in type 1 diabetes.

Abstract
Background: Diabetic disorders can lead to muscle atrophy. The aim of this study was to investigate the combination of resistance training and endothelial progenitor cell injection on the expression of horseshoe muscle atrophy factor in diabetic rats.
Methods: 30 rats (6 weeks old weighing 200 20 200 g) were randomly divided into five groups: healthy baseline, control diabetic, trained diabetic, non-trained diabetic by endothelial progenitor cell injection, diabetic trained by simultaneous injection Endothelial progenitor cells were divided. Heat 25 was measured by Western blotting to evaluate changes in protein expression. Data were analyzed by two-factor analysis of variance test by SPSS software version 19 at a significance level of α≥ 5%.
Results: In this study, 6 weeks of resistance training had no significant effect on the expression of heat shock protein 25. But injection of endothelial progenitor cells resulted in a significant increase in the expression of heat shock protein 25. The interactive effect of resistance training and progenitor cell injection on heat shock protein 25 was not significant, in other words, there was no significant superiority over training and injection at the same time as training or injection alone.
Conclusion: Based on the findings of this study, it can be stated that injection of endothelial progenitor cells can improve muscle wasting but resistance training alone was not effective. Also, the combination of these two strategies was not superior in reducing the complications of atrophy in diabetic rats.
